Growth factor receptors, such as HER2, are often activated in tumour cells and provide targets for specific cancer therapy. In this study, we examined the HER2 status in patients with advanced colorectal cancer to determine its clinical implication. HER2 overexpression was examined by immunohistochemistry (IHC) in 160 samples of paraffin-embedded tumour tissue from patients with advanced colorectal cancer treated with cetuximab and chemotherapy. DNA was extracted from tissue samples and analysed for HER2 amplification using fluorescence in situ hybridization. HER2 overexpression was observed in 20% (31 of 160) of the tumours. Amplification of HER2 was observed in 14% (22 of 160) of the tumours. Patients who did not respond to treatment had significantly more HER2 overexpression and amplification than did those who did (50 versus 18%, P = 0.01; and 28 versus 8%, P = 0.03, respectively). HER2 overexpression and amplification were found more frequently in mucinous than in adenocarcinoma (37 versus 15%, P = 0.01, and 19 versus 6%, P = 0.03, respectively). Patients with HER2 amplification had significantly poorer prognoses than did those without (median survival, 6 versus 13 months, P = 0.04, respectively). Survival of patients who received chemotherapy combined with cetuximab was significantly longer than that of patients who received chemotherapy alone (P = 0.02). Moreover, when HER2 overexpression and amplification were excluded from the analysis, the difference between the two groups became statistically insignificant (P = 0.3).
